1. If you need to cancel a family member's account that is incapacitated or has passed, the fastest way to begin the process is to call T-Mobile directly at 1-877-746-0909. Please note, there may be a possibility for you to keep the number.

2. For the quickest resolution, Sprint recommends you have the following information ready when contacting:

   1. Name.

   2. Name of the person on the account

   3. Their mobile phone number

   4. Date of death

   5. Last four digits of the deceased customer's Social Security number

3. You will be asked to provide valid documentation. They will provide information as to where to submit the following

   1. Copy of death certificate

   2. Electronic link to memorial website (Authorized users only)

   3. Obituary electronic or paper (Authorized users only)

   4. Copy of memorial (Authorized users only)

   5. Attorney / legal estate documents

   6. Cremation documentation

   7. For military service members. DD Form 1300 (Report of Casualty) is acceptable

   8. Other reasonable documentation

   Along with the following:

   1. Name on the account

   2. Account number and/or mobile number, see Find your account number

   3. Name of deceased, their relationship to you, and your contact information

   4. Social Security number, if available

   5. Company letterhead (if it's a business account)
